Understanding Our Need

Hospice and Palliative Care of Northern Colorado, Inc. seeks financial support to accomplish our mission of honoring and dignifying life’s final journey by providing quality end of life care, consoling the grieving and educating our community.

Hospice and Palliative Care of Northern Colorado receives reimbursement for our costs from Medicare, Medicaid, and private insurance. All three sources pay preset amounts on a per-day, per-patient basis. That rate, however, falls short of the actual cost of care. In fact, our costs per patient day run nearly $12.00 short of being covered by our reimbursements from Medicare, Medicaid, and private insurance. Each year we also provided nearly $150,000 in free care to families without the ability to pay.     

Perhaps more than any other medical care service, hospice is an all-inclusive benefit.  For one Medicare reimbursement...

• Hospice provides a terminally ill patient with care by physicians, nurses, home health aids, social workers, chaplains, and volunteers. 
• With virtually no co-pay from the patient, hospice provides all of the prescription drugs, over-the-counter medications, medical supplies, and home medical equipment necessary to provide treatment for the patient’s primary diagnosis. 
• Without any co-pay from the patient, hospice provides respiratory therapy, occupational therapy, physical therapy, speech therapy, and the services of a dietician when called for in a patient’s plan of care. 
• Hospice is also the only Medicare benefit that includes the patient’s loved ones in care by providing bereavement services for up to thirteen months after the patient’s death.

Sponsorship support is greatly appreciated and acknowledged several ways, depending on the level of sponsorship. We have several sponsorship levels:

• Crescent Sponsorship - $200 to $499

• Shasta Sponsorship - $500 to $999

• Swallowtail Sponsorship - $1,000 to $1,999

• Angelwing Sponsorship - $2,000 to $4,999

Monarch Sponsorships are included as sponsors of all events for a twelve month period.

All gifts and sponsorships of $500 or more are publicly recognized each January in an advertisement in both The Greeley Tribune and The Fort Morgan Times and in our Annual Report, released in November.
