Hospice and Palliative Care of Northern Colorado, Inc. is committed to educating the public in Weld and Morgan Counties about hospice care and end of life issues. We have a team of specialists available to make presenations to you, your workplace, social group, club or organization. HPCNC supports the use of 5 Wishes. This is an easy to use booklet that all people over the age of 18 should take the time to fill out. It spells out what you'd like the end of your life to look like. This booklet ensures that your wishes will be carried out, in the event you are unable to communicate your wishes to family and/or helathcare workers. HPCNC is also availble to respond to workplaces, schools or organizations following a death. Our organization employs trained bereavement workers, who have extensive experience in grief counseling.
