____________________________________________________________________ HPCNC has played an important role in the communities of Weld and Morgan Counties for over 25 years. Year after year our customer service remains a hallmark of our organization. When people hear hospice, our organization comes to mind. Hospice and Palliative Care of Northern Colorado, Inc. serves the rural and urban communities of Colorado’s Weld and Morgan Counties, a geographic area covering 5,293 square miles with a population over 220,000. Our services are tailored to meet the demographic profile of our area, which encompasses a diverse economic and minority population. Each community’s needs are met without regard to race, color, religion, age, gender, sexual orientation, handicap (mental or physical), place of national origin, diagnosis, or ability to pay. Our staff and volunteers’ dedication has been recognized both locally and nationally. One of our nurses received a nomination for the Nightingale Award, an award founded in 1985 to honor nurses who best exemplify the philosophy and practice of Florence Nightingale, a 19th century nursing pioneer. The award is sponsored by the Colorado Nurses Foundation and the Nightingale Statewide Selection Committee. Three HPCNC volunteers have been awarded the Weld County Stars of Excellence Volunteer Award in the health category administered by Weld County, and the 2006 United Way Amazing Volunteer of Excellence in the adult category was a hospice volunteer. Mission Vision Values Our Hospice Philosophy Since 1978, families in Weld and now Morgan Counties have turned to Hospice and Palliative Care of Northern Colorado, Inc. at a time when having the support and expertise of hospice staff is profoundly important. We provide care in patients' homes, in long-term care facilities and at our Inpatient Unit, located on the fourth floor of North Colorado Medical Center. As a comprehensive program, hospice care considers the unique physical, emotional and spiritual needs of each individual. Working closely with patients, families, medical professionals and caregivers, our expert and caring staff focuses on helping people who are terminally ill live with symptoms under control, in comfort and dignity. When the time of passing is near there are many signs and symptoms that our nursing staff can help family members identify and become aware of. The hospice philosophy was summed up by Dame Cicely Saunders (founder of the first modern-day hospice in London) who said to patients: "You matter to the last moment of your life, and we will do all we can, not only to help you die peacefully, but to live until you die."
